
Please read and complete all required forms (available on the Forms) page and bring them with you to your scheduled appointment along with your insurance card(s) and a valid form of identification.
Workers Compensation Patients: If you are making an appointment under a Workers Compensation Claim, your case worker will need to contact our office with all needed claims filing information immediately, as failure to have this information may slow down the process of your care.
Referred Patients: We accept Medicare and most major managed care plans. Some managed care plans require referral authorization from your primary care provider prior to receiving services from a specialist. If your plan requires a referral authorization, please obtain it prior to your visit.
Self-Pay Patients: If you are currently uninsured and will be seeing Dr. Littlefield as a “Self Pay” patient, please contact our Office Administrator at 678.608.1951 prior to your appointment so we may assist you with your financial needs.
Treatment of Minors: This practice will treat children of all ages for injuries and illnesses, provided the child is accompanied by a parent or legal guardian. (No exception).
Policy On Controlled Substances (Pain Medication) Order/Refill Requests: It is the policy of this practice to only honor narcotic (pain) medication orders or refill requests during regular office hours. No controlled substances (pain medications) or refills will be ordered or called into your pharmacy after hours or on weekends.
Appointment Scheduling (PLEASE READ CAREFULLY): Because we value your time we have a “Two-Time” appointment system. The first time that we will assign you is the “Show” Time: the time that you need to arrive at our office, to allow us time to process your paperwork, obtain medical tests, take x-rays, etc.
The second time we will assign you is the “Room” Time: the time we expect to place you in an examination room to see Dr. Littlefield. Dr. Littlefield strives to see you within 5-20 minutes from the time you are placed in an exam room. It is more important to afford you the time you need with Dr. Littlefield, rather than spending less time with you to stay "on schedule." However we do plan, and do everything we can, to stay "on schedule" because we know your time is valuable. Please understand that if we are running late, you too will get every minute that you need with Dr. Littlefield during your visit.
