New Patient Forms Package -- Interactive PDF Format

Instructions for completing the interactive PDF form:

This form can be completed electronically and then printed, or printed blank and completed in pen. The completed form should be faxed to our office at 678.608.1952 prior to your appointment, or you may bring it with you to the appointment. Please follow the steps below:

  1. If you do not have Adobe Reader installed on your computer you will not be able to open and complete the form. You can download a free copy of Adobe Reader if needed at http://get.adobe.com/reader/
  2. If the link does not open the file automatically, please try this: right-click on the link, select “Save link as …” and save to the location of your preference on your computer (your Desktop or My Documents folder are recommended locations). If you are still unable to open the file, you may need to download and install a more recent version of Adobe Reader.
  3. After saving the file to your computer, open Adobe Reader, then go to File > Open and locate the saved file. If you do not see blue field highlights when you open the form, click on the Highlight Fields button found at the top right. If you would prefer, you can go to File > Print and complete by hand with a pen.
  4. Completed all highlighted fields. You can use the Tab on your keyboard button to go to the next field. There are seven (7) pages in the form.
  5. There are several fields that require a digital signature. If you do not already have one, the form will take you through the steps to generate one when you click on the first signature field. You will need to enter some basic identifying information (name and email address) when prompted and create a password, then you will be prompted to save the signature file on your computer. After going through these steps, you will need only to select the signature and enter the password when prompted as you complete each signature field.
  6. After completing the form, you have two options:
    • Fax to our office at 678.608.1952.
    • Bring the form with you to your appointment.
